There are two modes of: patient and doctor.  The doctor interface is more 
fully developed, and will actually ask all of the questions for Bulimia 
Nervosa.  If you login as a doctor, the program will accept any 4-digit number 
as a valid license number.  

For both interfaces, spelling and capitalization matter and if you mispell any 
response you will simply be asked to re-enter.
